    Tirzepatide dosing at a glance

    • How often: Once weekly.
    • Typical run: Long-term; 0.5mg steps only as needed.
    • Vial sizes: 10mg, 20mg, 30mg. Formats: Vial / Pen.
    • Starting dose: 0.5mg, stepping up 0.5mg at a time only if needed. Top dose on this site: 15mg (the top dose studied in the SURMOUNT trials (the highest Zepbound® dose)).

Start at 0.5mg once a week, on the same day each week, and step up in small 0.5mg amounts only if you need to. Stay at least two weeks on each dose, and longer if you have side effects. Step up only if weight loss has slowed to less than about 1–2 lb a week. Once you are losing steadily, stay at that dose: there is no need to keep climbing. Many people find their dose well below the top of the ladder.

Tirzepatide units on a U-100 syringe

With a 10mg vial and 2ml of bacteriostatic water the mix is 5mg per ml, so every 10 units holds 0.5mg. Change the vial or the water in the calculator above and the units update.

Tirzepatide schedule: guided, not on a timetable

Start at 0.5mg and stay at least two weeks on each dose, longer with side effects. Step up 0.5mg only if weight loss has slowed below about 1–2 lb a week; once you’re losing steadily, stay there. The ladder runs 0.5mg → 1mg → 1.5mg → 2mg → … up to 15mg, or Dial 5 on the pen.

    Common questions

How many units is 1mg of Tirzepatide?

With a 10mg vial mixed in 2ml, 1mg is 20 units on a U-100 insulin syringe. With 3ml of water it would be 30 units: more water means more units for the same dose, which is easier to measure for small doses.

How much bacteriostatic water do I add to a 10mg vial of Tirzepatide?

2ml is the common default; anywhere from 2ml to 3ml is fine. More water doesn’t change the dose, only how many units it takes to draw it. See mixing a vial.

What if I miss a Tirzepatide dose?

Never double up. For a weekly peptide, take it if the next dose is still a few days away; otherwise skip it. The missed doses guide has the rule for each peptide. Dose-day emails above come with an “I took it” button so you never have to wonder.
